We are pleased that you have selected the Athletic Institute of Medicine, the leader in sports medicine in Arizona, for your Orthopedic and Sports Medicine care. In order for us to provide you with the highest standards of care, we must ensure that we can cover our expenses. Therefore, it is important that you understand your financial liability.
Prior to being seen, you will be required to present your current insurance card. At that time you will be informed of your financial responsibility due to co-payment, co-insurance on deductibles not covered by your insurance. These responsibilities must be paid in full prior to being seen by the physician or Physical Therapist.
If you are unable to present your insurance card we will make every effort to contact your insurance company and verify your eligibility. If we are unable to do so you will be required to make full payment in order to be seen.
If you are an established patient, it is your responsibility to notify us of any changes to your insurance coverage. The Athletic Institute of Medicine will try to verify your benefits prior to your services. However, it is ultimately your responsibility to make sure we are a provider and you are aware of all your benefits and insurance limitations.
We will file your claim with your insurance company. In the event your claim is denied, you will be responsible for payment in full for the services you received.
Prior to any surgical procedures, you will be required to make full payment of your deductible or co-insurance responsibilities.
For self-pay patients, we offer a discounted fee schedule for cash payments. For all other patients, we accept most major credit cards, cash and personal checks. Please note, there will be $40 charge for all returned checks.
